Participants, as well as parents or guardians, understand that publicity photographs may be taken during the tournament and their image/photographic likeness may appear in photographs taken during a variety of related events. By registering to volunteer for the Overton Park Junior Open (OPJO), you hereby give permission that photos containing your image/likeness may be used for publicity or general information purposes including on the OPJO website or within other publicity or related publications or media which may be seen by the general public. The OPJO will not seek any further permission nor provide any notification before using such photos.

Assumption of RiskRequired. If volunteer is under the age of 16, a parent or guardian must fill out this form, approve for his/her child to volunteer for OPJO activities, and agree to assume all risks and waive liability accordingly.

In volunteering for the Overton Park Junior Open (OPJO), I understand and assume the risk and danger incidental to the game of golf, including, but not limited to, the risk of being hit by an errant or misdirected golf shot. I assume all risk or injury whatsoever and agree to hold harmless: Overton Park Junior Open golf tournament, Overton Park Conservancy, Memphis Parks, and the City of Memphis, and the agents and employees thereof, from any and all liability from such causes that might occur while volunteering for the OPJO, and from claims of any nature arising from any activity associated with the OPJO. This hold harmless agreement includes, but is not limited to, any claim due to injury resulting from negligence of the OPJO, Overton Park Conservancy, Memphis Parks, and the City of Memphis, including employees, participating agencies, and volunteers. This consent form is valid for all OPJO activities for the calendar year of 2023.
